FREE SPORTS PHYSICALS OFFERED FOR CALHOUN ATHLETES - Two Dates Announced

Minnie Hamilton Health Care Center will provide free sports physicals for all students who will participate in sports and cheer-leading during the 2003-2004 school year, as required by the West Virginia Secondary School Activities Commission (WVSSAC).

Sports physicals will be provided at the School Based Health Center at Calhoun Middle/High School on Thursday, July 17, from 8:00 a.m. until 4:00 p.m.

If a student is not able to come on that date, sports physicals will be provided at the same location on Tuesday, July 29, from 8:00 a.m. until 12:00 p.m. Two providers from Minnie Hamilton Health Care Center will be on-site.

Students who are not able to have their sports physicals on July 17 and July 29 may do so at Minnie Hamilton Health Care Center at any time during the summer.

As with all services provided by the School Based Health Center, the student's family will not be billed for the sports physical.

If a student is covered by private insurance, WV CHIP or Medicaid, those carriers will be billed directly. The student's family will not be responsible for co-payments or deductibles.

The WVSSAC consent form must be completed and signed by a parent or guardian prior to the student receiving the sports physical. The consent form may be picked up in advance at the following locations: Minnie Hamilton Health Care Center, J&B Drug Store, Rite Aid of Grantsville, Speedy Mart, and Holbert's Store.
