MHHS SPORTS PHYSICAL CLINICS PLANNED FOR CALHOUN-GILMER COUNTIES |

| GILMER COUNTY - The Minnie Hamilton Health System Glenville Office will be offering a free sports physical clinic on July 15th from 8:00 a.m. to 6:00 p.m. at the Gilmer County School-Based Health Center at Gilmer County High School. Insurance will be billed and any deductible and/or co-payment amounts will be written off. If the sports physical is not completed at the School-Based Health Center on July 15th, the cost of the physical will be $57. Insurance will be billed and any deductible and/or co-payment amount will be the responsibility of the parent/guardian. If you do not plan to attend the clinic with your child, you must complete a sports physical form and sign the consent for treatment. No physicals will be given without a signed parental consent. In addition to a signed consent, your child must have a copy of any insurance card(s). Scheduling an appointment is the only way to guarantee a completed sports physical on one of the sports physical clinic dates. Walk-ins are permitted but will only be given a sports physical if time allows. Appointments may be made by calling 304-462-7322. CALHOUN COUNTY - Sports Physical Clinics will be held Monday and Tuesday July 14th-15th 7:30 a.m. to 5:30 p.m. at the Calhoun Middle-High School in the School Based Health Clinic. Parent will need to fill out a form prior to the athlets appointment. Forms can be picked up at Minnie Hamilton Health Systems front desk or in the office at the school.
