| Submitted by Kelly Yoak Are you ready for some football? The first round of sign-ups has been scheduled. Friday, June 7th at the Grantsville fire hall from 6-7:30 p.m. Saturday, June 15th at the Middle/High School baseball field (look for Kerri Stull) from noon to 3 p.m. Documents needed at time of sign up. If you do not have all documents please wait until another sign up date. If all documents are not turned in at sign-up your child's sign-up form will not be accepted until everything is there. Recent picture of child. Not over a year old. Last report card of school year. MUST BE LAST! $30 fee for tackle football and cheer $20 for flag football and flag cheer. Yes we are having Flag Cheerleaders. (ages 3-Kindergarten) Birth certificate for flag. Does not have to be certified but must be copy of original birth certificate. Not hospital issued. Again all documents need to be turned in at time of sign-ups. Sports physicals need to be turned in at first day of practice which will be Monday, July 22nd. MUST BE SPORTS PHYSICAL not Well Child checkup form. All flag and tackle Football players and cheer need a sports physical. If your child has had one already this year for another sport you can get a copy of that. No need for another but has to be this year 2013.
