SCHOOL SPORTS PHYSICALS AND IMMUNIZATIONS CLINIC PLANNED FOR JUNE

Calhoun County School Based Health will be offering sports physicals at Calhoun County High School in the School Based Health Clinic on Monday June 13th, Tuesday June 14 and Wednesday June 15th from 8:00 a.m. to 6:00 p.m. Well Child Checks (including any needed immunizations) will also be done at this time if you child has not had one in the past year. Parents will be asked to provide proof of the Well Child Check if one has been done in the past year with a provider other than Minnie Hamilton. Your insurance will be billed, any remaining amount will not be the responsibility of the patient/parent when seen on one of the above sports physical clinic dates.
