(08/18/2012)
Students entering the 7th or 12th grade this fall must have additional immunization, according to CM-HS principal Karen Kirby, who has issued a reminder to parents.

Minnie Hamilton Health System School Based Health Center (SBHC) at Calhoun Middle-High School will be open on August 23rd through 30th to administer the required 7th and 12th grade age-appropriate immunizations, Tdap (tetanus, diphtheria, pertussis) and MCV4 (meningitis).

Students are being allowed a two-week grace period to provide documentation of the required vaccinations. Parents are reminded to pick-up a consent form from the SBHC beginning August 23rd or from Minnie Hamilton Health System (MHHS) clinic anytime and return to the SBHC for their child to receive the immunizations if needed.

The SBHC clinic will resume normal hours 8:00 - 4:00 at the CMHS beginning September 4, 2012.

Regular hours with a provider are: Monday 8-4; Tuesday 8-11:30; Wednesday 12-4; Friday 8-4.

If you have further questions contact Stacie Starcher 304-354-6851 starting 8/23/12 at the SBHC or Nina Smith, RN at 304-354-9244.
