The incident was about two miles from Arnoldsburg.
Reports indicated the vehicle was a total loss.
The Upper West Fork VFD answered a mutual aid request to a car accident at the head of Mud Fork in the edge of Clay County at 9:22 p.m. Friday.
The request for assistance came from the Big Otter VFD, after a car went over an embankment
