MINNIE HAMILTON ISSUES NEW SECURITY POLICY

Minnie Hamilton Health System values the privacy of our patients and is committed to maintaining the security of patient information. We must take precautions to ensure the security of your identity and your health information. To be in compliance with federal law, effective November 1, 2009, we must request that you provide proof of identity by bringing the following information with you to your next visit: • A current Driver's License or Photo ID • A current Health Insurance Card if you have one; and • A utility bill or other correspondence showing current residence if your license or photo ID does not show a current address. If the patient is a minor, the patient's parent or guardian shall bring the requested information.
