SPORTS PHYSICALS OFFERED AT SCHOOL BASED CLINIC

Submitted by Barb McKown A sports physical is required for all students who plan to participate in any school sports program next year. Sports physicals are required yearly and will be available at the School Based Clinic May 27-30th. Students must obtain a physical form, and have a parent complete and sign the form. Bring the completed form to the clinic to schedule an appointment. You must have a physical before team practice begins and you must have a completed form to have a physical. Forms are available from the coaches and are also available at the School Based Clinic.
